• Your institution and its technology-oriented programs
• Any efforts you have to support technology-based entrepreneurship
• Contacts at your institution who are responsible for such activities, and the name and contact information for your preferred primary contact
• How you support your entrepreneurship efforts; sponsorship from industry, government or other
• A short history of your entrepreneurship efforts and any notable results
